ON the modern stuff, you use spacers to raise and lower the stem on the steerer, but most people chop off the excess steerer tube, removing any future adjustability. More often now, you buy stems with different lengths and vertical offsets.
The old quill stems were much better for town bikes and general riding-you could adjust the height to suit the ride.

On newer bikes with threadless headsets you can sometimes make minor adjustments by moving spacers above/below the stem. Also you can get stem extenders if you want a lot more rise.
("vintage"?! ...I guess I do favor older bikes, but I didn't realize threaded headsets had entered the realm of "vintage." You're making me feel old, here! π)
